Guide: Schwerbehindertenausweis und Nachteilsausgleich (Severe Disability Benefits) in Germany
What is it?
The Schwerbehindertenausweis is an official identification document that confirms a person has a severe disability with a GdB (Grad der Behinderung – degree of disability) of 50 or more. It serves as official proof of severe disability and provides access to various compensatory benefits (Nachteilsausgleiche).

Who is eligible?
You are entitled to a Schwerbehindertenausweis if you meet the following criteria:
  • Degree of disability (GdB) of 50 or more – degrees range from 20 to 100, but only GdB 50 and above grant "severe disability" status.
  • Long-term disability – the condition must have lasted or be expected to last at least 6 months.
  • The causes can be any: chronic illnesses, accident consequences, mental disorders, congenital defects, etc.
  • You live in Germany with a registered address or habitual residence.
Important changes from 2026: Since January 1, 2026, the procedure for proving disability for tax purposes has changed. The responsible authorities now transmit the assessment data (GdB and Merkzeichen) electronically to the tax office. To enable this automatic data transfer, you must provide your tax ID number (Steuer-Identifikationsnummer) and give your consent for data transfer when submitting your application.
Application process:
  • Submit your application to the Versorgungsamt (or Amt für Soziales) at your place of residence.
  • You can apply online, by post, or in person.
  • You need to provide comprehensive medical documentation: doctor's reports, hospital records, specialist letters, X-ray findings, etc.
  • The application is free of charge.
  • Processing time: Usually 4 months on average , though some sources mention 6-8 weeks after receipt of your photo. The duration depends heavily on how quickly doctors and hospitals respond to the authority's requests for medical findings.
Note: If your GdB is less than 50, you will still receive a notification (Feststellungsbescheid) with the determined degree, but no Schwerbehindertenausweis will be issued.

Value stamp for free public transport
To use public transport for free, you need the "value stamp" (Wertmarke) on the supplement sheet (Beiblatt).
Cost of the value stamp (2026):
  • Free: For Merkzeichen H and/or Bl 
  • Free: For recipients of social assistance (SGB II, SGB XII) 
  • Paid: For Merkzeichen G, aG, and/or Gl – €53 for 6 months or €104 for 12 months 
The Merkzeichen "B" must be specifically applied for in your initial or renewal application and is subject to individual case assessment.

Deadlines and important dates
  • When to apply: The application can be submitted at any time, as soon as you have confirmed medical diagnoses.
  • How long to wait: Processing usually takes about 4 months  (sometimes 6-8 weeks after photo submission ).
  • Validity: The ID may be issued indefinitely (unbefristet) or with a review date (e.g., after 3 or 5 years) if the condition might improve.
  • Renewal: You can apply for renewal 4-6 weeks before the ID expires. If only the supplement sheet (Beiblatt) with value stamp expires, renewal may be done without a new full application, provided a new valid social benefit notice is submitted.
  • If you disagree with the decision: You have 1 month to file an objection (Widerspruch) from the date you receive the decision.
Typical mistakes
  1. Applying without sufficient medical documentation. The most common reason for a low GdB or rejection. Gather all doctor's letters, reports, examination results.
  2. Underestimating the importance of Merkzeichen. Sometimes a GdB of 80 without the "G" letter means you won't get mobility benefits. Ask the authority to check if you are eligible for the marks.
  3. Missing the Widerspruch deadline. One month passes – the decision becomes final and very difficult to challenge later.
  4. Not knowing you can apply for a GdB increase if your condition worsens. If your health deteriorates, go to the Versorgungsamt with new documents – your GdB may be raised. You can submit a so-called "deterioration application" (Verschlechterungsantrag) at any time, even if you have already filed an objection or a lawsuit.
  5. Not using tax benefits. Many receive the ID but forget to file a tax return with the Pauschbetrag. That is hundreds of euros per year.
  6. Going to the wrong authority. The Versorgungsamt (or social welfare office) at your place of residence is responsible, not the health insurance fund (Krankenkasse) or pension insurance (Rentenversicherung).

The responsible authority here is not the pension fund or the health insurance fund, but the Versorgungsamt (Social Welfare and Supply Office) of your federal state. The ID card is a document that records your status and grants specific rights in everyday life (taxes, transport, parking).

Step-by-step instructions

Step 1. Understand the difference between degrees.
First of all: a doctor's certificate confirming a diagnosis and a disability ID are not the same thing.
  • GdB (Grad der Behinderung) – the degree of functional impairment, measured on a scale from 20 to 100.
  • Severe disability (Schwerbehinderung) begins at GdB 50 or higher. Only from this threshold do you receive the actual yellow-green plastic card (Ausweis) and the right to Nachteilsausgleiche (compensatory benefits).
If your GdB is below 50, no ID card is issued; you will only receive a written decision (Feststellungsbescheid). This can be useful for tax purposes (from GdB 20 onward), but it does not grant transport or parking concessions.

Step 2. Complete the application form (Antrag auf Feststellung einer Behinderung).
Forms are available from the Versorgungsamt of your city or district (sometimes integrated into the Bürgeramt or Landesamt für Soziales). You can also download the form from your federal state's official portal.
In your application, you are not asking for "disability" in the abstract. You should write: "I hereby apply for the determination of my degree of disability (GdB) and for the issuance of a severely disabled person's ID card with the appropriate supplementary marks (Merkzeichen)."

Step 3. Prepare the attachments: list of physicians and consent to data collection.
This is the main bureaucratic mechanism. The Versorgungsamt will not examine you in person (in 90% of cases). Instead, it will request reports from all the physicians who treat you.
In your application, you must provide:
  • The full name, address, and specialty of each physician you are seeing (general practitioner, orthopedist, cardiologist, psychiatrist, etc.).
  • Dates of outpatient and inpatient treatments.
You must also sign a consent form (Schweigepflichtentbindung) authorizing the authority to collect this medical information. Without this, your application will not be accepted.

Step 4. Monitor the requests (in your own interest).
The authority will send questionnaires to your physicians. Make sure they respond. Delays are often caused by your own doctor's silence. I recommend informing your physicians in advance: "You will receive a request for a medical report (Befundbericht) from the Versorgungsamt – please respond promptly and in detail." In their response, the doctor should describe not just the diagnosis in Latin, but the functional limitations (e.g., not "stage 3 osteoarthritis," but rather "can walk without pain for only 200 meters and requires a walking aid").

Step 5. Await the decision and review it carefully.
The decision arrives as a Bescheid (official notice). It will state your overall GdB (a single combined figure, not a sum of individual conditions). If you disagree with the number (e.g., they gave you 40 but you believe it should be at least 50), you have 1 month to file an objection (Widerspruch). Your objection must include recent medical documents that the authority has not yet considered.

Step 6. Receive your ID card and apply for supplementary marks (Merkzeichen / Nachteilsausgleiche).
If your GdB is 50 or higher, you will be sent or offered the ID card. The most important part is the supplementary marks on the reverse side (Merkzeichen) . These are not granted automatically with the GdB – they must be separately justified with medical documentation. The main ones are:
  • G – Gehbehindert (mobility-impaired): Grants a reduction in vehicle tax and concessions when using public transport. Basis: you cannot walk more than 2 km on level ground without interruption.
  • aG – außergewöhnlich gehbehindert (exceptionally mobility-impaired): Grants a blue parking permit (Parkausweis blau) and full exemption from vehicle tax. Basis: you can only move with the help of others or with great difficulty, comparable to being unable to walk.
  • B – Begleitperson (need for an attendant): Grants the right to free travel for an accompanying person on trains and public transport. Basis: you require assistance when using transport (e.g., blindness, severe psychiatric disorders, seizures).
  • H – Hilflos (helpless): Basis for a pension supplement or care-related tax deductions. It means you need help with basic daily activities (eating, hygiene) to survive.
  • RF – Rundfunkbeitragsbefreiung: Exemption from the TV/radio license fee.
How to use these entitlements:
  • If you are awarded GdB 50, you automatically have the right to a tax allowance (Pauschbetrag) and enhanced protection against dismissal (Kündigungsschutz) .
  • If you receive the G or aG marks, take your ID card to the Bürgeramt or the Road Traffic Office (Straßenverkehrsamt) to obtain a parking permit or to apply for vehicle tax relief.
  • If you receive the RF mark, send a copy of your ID card to the fee collection service (ARD ZDF Deutschlandradio Beitragsservice) to cancel your license fee payments.

If the government agency requests additional documents

When you have submitted your application and the Jobcenter has already partially reviewed it, they may request additional documents. A serious mistake is to simply forward the requested documents without a cover letter.
In such cases, the officials at the Jobcenter do not understand which case the documents belong to, cannot link them to a specific file number, consider the deadline not met, and issue a rejection stating „Unterlagen nicht fristgerecht eingereicht“
